Intensive Outpatient Program near Spring Hill, FL
From Spring Hill, Karuna’s Tampa office on Sheldon Road is a long run down the Suncoast Parkway, and three round trips a week is a lot to ask of anyone already stretched thin. The ten-week program runs three ways — 100% by telehealth, hybrid, or fully in person — so Spring Hill adults can complete the whole intensive outpatient program without making that drive at all.
It is one program rather than a menu: three days of group therapy per week, one individual therapy session per week, family therapy when it helps, and an optional alumni program afterward, all for adults 18 and older. In the words of CEO Harrison Spielman, “We don’t offer a magic pill. We don’t offer a magic solution, but we offer those skills.”

Knowing What Follows the First Call
- Online forms get a reply within one business day; calls are answered day or night
- A short pre-assessment about what has been going on — you do not need to have every detail figured out
- Coverage checked and your deductible explained in plain terms, since with in-network insurance it is the only part you pay
- Your assessment and first group, which can both happen on the day you call if you feel ready
Honest Talk About the Deductible
For Spring Hill households, paying for the program starts with two in-network plans: Blue Cross Blue Shield and TriCare, the latter a real help to the veterans and military families Karuna works with. With either, you are responsible for the deductible only, and past it the program is covered. Every policy sets its own deductible, which is why the team looks yours up rather than printing a number here. Private pay is accepted, and payment plans are available.
Anything you share with the team stays private. If you are in danger or thinking about harming yourself, please call or text 988, or call 911; an outpatient program is not emergency care.
Understanding What the Ten Weeks Work On
An IOP sits a full step above weekly therapy: three days of group therapy per week, one individual therapy session per week, and steady practice in between, while you keep sleeping in your own bed. People tend to arrive with high depression, high anxiety and social isolation. Across ten weeks, the work is to break those walls down, see what is driving the symptoms, and build skills for coping with them.
Anxiety treatment, depression and mood disorder care, OCD work, trauma support and help with the isolation that often comes along all happen inside the one intensive outpatient program open to Spring Hill adults. None of it is sold as a separate track; it is a mental health program from start to finish.
Attentive Care From the People Who Own the Clinic
Owners Who Show Up Every Day
Karuna opened in June 2024 and is clinician-owned: co-owner and clinic director Dylan Hollander built the clinical program in-house from the ground up. Harrison Spielman describes it this way: “We’re a small bootstrapped business that’s run by the ownership — we’re here every day and we’re here for the patients.”
Laptop or Office Chair, Same Small Group
People on video and people in the Tampa office sit in the same group, capped at 16 combined and usually 12–14, so logging in from a kitchen table off Mariner Boulevard still means a small, steady circle rather than a webinar.
Extending Past Week Ten When It Helps
There is no intake calendar to wait on, ten weeks is a minimum commitment rather than a cap, and you graduate when you are ready. If the work is still helping, insurance can usually be arranged to accommodate a longer stay.
Careful Design Behind a Program Spring Hill Adults Can Reach
The clinical framework rests on the 7 Dimensions of Wellness — physical, emotional, intellectual, social, spiritual, occupational and environmental — as a foundation rather than a weekly schedule. The methods are evidence-based: Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Exposure and Response Prevention, which the whole clinical staff is proficient in.
- Three days of group therapy per week, Monday, Wednesday and Thursday, either 9:00am–12:00pm or 6:00pm–9:00pm
- One individual therapy session per week, typically 45 to 60 minutes, which may fall inside or outside group hours
- Ten rotating themes in no set order, from boundaries and codependency to grief and loss, so any week works as a first week
- Symptoms measured at four points: intake, month 1, month 2 and discharge
- An optional alumni program after week ten, with check-in calls and an open line back to your former therapist
Nothing is thinned out for video: whether you join from home or walk into the Sheldon Road office, the program Spring Hill adults attend runs on the same schedule, themes and measurement points. Just across County Line Road, Karuna’s behavioral health services near Pasco County run on this same IOP, and telehealth reaches statewide across Florida.
Spotting the Point Where Once-a-Week Therapy Stops Holding
- Work is getting unreliable — late starts, missed shifts, or a commute you dread more each week
- Sleep, parenting or basic follow-through is getting harder to hold together, even though you are trying
- Anxiety or low mood has shrunk your days down to the house, the car and not much else
- Weekly therapy helps, but the gains fade long before the next appointment
- You are unsure which level of care fits — sorting that out with a clinician is exactly what the pre-assessment is for
Avoiding Guesswork About Joining From Spring Hill
Do I ever have to make the drive from Spring Hill to Tampa?
No. The intensive outpatient program can be completed 100% by telehealth from Spring Hill, so the trip to the Tampa office is a choice rather than a requirement — useful on summer evenings when storms are rolling through Hernando County. Hybrid and fully in-person attendance are open to you as well.
Is an IOP still a fit if I’m retired in Timber Pines?
No. The program is for adults 18 and older, with no upper age limit — or, as Harrison Spielman says, “Someone’s 100 years old and they want to come, they’re more than welcome.”
Can I keep my job in Brooksville or Tampa and still attend?
The evening track is there for exactly that: groups run 6:00pm to 9:00pm on Mondays, Wednesdays and Thursdays, and you can join from home once you are back in Spring Hill. Master’s-level clinicians Stephanie Johnson and Mia Martinez run the evening program, and a morning track meets 9:00am to 12:00pm on the same days — pick whichever fits your week.
Does it take a referral or a psychiatrist to get started?
Neither. No referral is required, and starting without ever having taken medication or seen a psychiatrist is common. Any medication stays in the hands of the doctor who already manages it — Karuna has no prescriber in the building — and the team works alongside that doctor, or helps you find a psychiatrist if you want one.
How much would I pay out of pocket with in-network insurance?
With Blue Cross Blue Shield or TriCare, your share is the deductible only, and everything beyond it is covered. Because each policy carries its own deductible, a call to (813) 210-7300 is the honest way to learn yours. Private pay is also accepted, with payment plans available.
